Objectives: Bleomycin is a chemotherapeutic agent with low toxicity to non-tumor cells. Tumor-specific electroporation (TSE) increase bleomycin uptake into tumor cells, potentiating its effect by up to 700-fold. Treatment of equine sarcoids (ES) with bleomycin TSE has not been described in large case series to date. Methods: Case records of horses presenting with ES and treated with intralesional injection of bleomycin followed immediately by TSE, either alone or in combination with surgical debulking, were reviewed. Cases with a minimum follow-up of 8 months were included. Results: Twenty-six horses met the inclusion criteria. A total of 66 lesions were treated, ranging from 1 to 10 lesions per horse. Lesions were located on the head (n=7), trunk (n=35) and limbs (n=24), and comprised 24 nodular, 12 occult, 11 verrucose, 10 mixed, and 9 fibroblastic lesions. Tumor volume ranged from 0.5 to 18.5 cm3. Most horses (n=23) were treated under locoregional anaesthesia and standing sedation, unless lesion location necessitated treatment under general anaesthesia. Horses received 1 (n=8), 2 (n=12), 3 (n=5) or 4 (n=1) TSE treatments at 4 to 6 week intervals. At eights months follow-up, complete remission of treated lesions was seen in 25 horses (64 lesions), resulting in a complete remission rate of 97%. In one horse (four lesions in total), two lesions were still evident after three TSE treatments. The only noted adverse treatment effect was non-painful local swelling which regressed within 2-3 weeks post treatment. Conclusions: Bleomycin TSE proved safe and effective in treating different types of equine sarcoids.
